What is housing case management?

Housing case management helps prevent homelessness by helping people keep their homes or find new housing quickly. Case managers are excellent collaborators and connectors with an in-depth knowledge of community resources and a sincere empathy for the needs of others so they can properly assess cases and coordinate services.

Who can benefit from housing case management?

Housing case management can make a difference for families or individuals trying to avoid homelessness or quickly find a new living situation when necessary.

Under the HPRP, a case manager can help if your income is at or below average for your area and at least one of the following applies to you:

  • You are sleeping in an emergency shelter
  • You are sleeping somewhere not intended to be housing, such as your car, a park, the street, or in abandoned buildings
  • You are staying in a hospital or another institution but were homeless beforehand
  • You are about to leave a transitional housing program
  • You are escaping domestic violence

What will my housing case management process look like?

Once you have a case manager, they will determine what kind of housing you need and what level of assistance is appropriate. The process is intentionally designed to be collaborative, working together to obtain housing for you with HPRP assistance.

Once you have stabilized your housing situation, your case manager can work with you to determine what you need to maintain stability, such as child care services, health insurance, benefits, or food or clothing.

HPRP focuses only on short- or mid-term needs, so if you or your family need ongoing services, your HPRP case manager can assist you by making appropriate referrals.
