Homelessness numbers are on the rise in the United States — between 2023 and 2024, numbers rose 18% from slightly more than 653,000 to more than 771,000.

When you’re facing housing insecurity

That feeling of not knowing where you’re going to find a safe place to sleep each night is an awful one, and one that’s experienced by:

• Hospitalized or institutionalized patients who have nowhere to go

What housing case management entails

As the name suggests, housing case management is a service that provides individualized oversight for your housing situation. If you qualify for housing case management, you’ll be assigned a case manager who will serve as your primary point of contact for housing security.

Your case manager will work with you on addressing your short- and mid-term housing needs so that you can get back on your feet again. And their work extends beyond housing and includes making sure you have access to necessary resources, such as:

• Healthcare and health insurance
• Child care
• Food 
• Clothing

Extensive resources to prevent homelessness

Your case manager has access to an extensive network of resources through California’s Homelessness Prevention and Rapid Re-Housing Program. This access allows them to work with you until your basic needs are covered and the threat of homelessness doesn’t hang over your head.

This effort is collaborative, which means your participation and input are key to the process. Think of your case manager as someone who can help you navigate your way out of housing insecurity and into a world that’s more stable, but you’ll need to take these steps once we point the way.
